Yes I think that is right! Just got mine and it has my Mom's maiden name even though she is married to my Dad. I think it's how it's listed on your birth certificate!

We're in NY and we just brought our license and birth certificates to the town clerk and they did all the work. They're very particular in NYS - so you don't want to risk filling it out incorrectly.
